Sundeep, a 63 year old office worker, presents with intermittent nagging loin pain and achy muscles. 

There is no abnormality on examination but he does have a history of hypertension and reflux which he takes medication for. 

He is generally well but something is not quite right. 

What is the most likely diagnosis? Which investigations are required to determine the potential underlying cause? Is treatment required and, if so, with what and for how long?

Sundeep has hypocalcaemia. From the GP's perspective it can be difficult to diagnose and treat.
