Caring for people living with dementia as a chronic disease

Overview

Almost half a million Australians are currently living with dementia, and this number is expected to nearly double by 2054 if current trends continue. As the second leading cause of death in Australia, dementia presents a significant challenge for general practice, particularly when compounded by other chronic diseases such as diabetes, cardiovascular disease and frailty.

This interactive, case-based webinar provides a comprehensive overview of current best practices for managing patients with dementia in primary care.

Why This Webinar Is Essential for General Practitioners

As the population ages, general practitioners and primary care teams are increasingly called upon to support people living with dementia across all stages of the disease. This webinar explores:

  • Dementia as a chronic condition requiring structured, long-term care planning

  • The impact of dementia on other chronic diseases, and vice versa

  • Coordinated care approaches that enhance quality of life and reduce carer burden

What You Will Learn

This session will help you:

  • Apply a multidisciplinary care approach to managing patients with dementia

  • Identify how cognitive impairment alters chronic disease management like diabetes

  • Understand the GP’s role in early recognition, regular review, and ongoing support

  • Enhance team-based care through better referral pathways and collaboration with allied health professionals, specialists, and community services

Case-Based, Practical Learning

Realistic case studies will bring the theory into practice, focusing on challenges and decision points faced by GPs and practice nurses. The webinar encourages interactive learning to equip you with the tools to improve care planning and health outcomes for this growing patient population.
